What Does Post-Hospital Care at Home Include?

Post-hospital care is tailored to the individual’s recovery goals and risks. Support may include nurse-led monitoring, medication management, mobility support, nutrition and hydration, and care coordination with GPs and therapists.

For more complex needs, our Palliative & Complex Care hub may be helpful.

What Does Recovery Care Look Like in Practice?

Example 1: Mrs K returned home after hip replacement surgery. Support focused on safe mobility, daily exercises, medication reminders, and personal care. Within weeks, she regained independence and avoided complications.

Example 2: Mr J was discharged after pneumonia with reduced mobility and fatigue. Nurse-led care monitored oxygen levels, reviewed medications, and helped his family create a safer home environment.

When Is Post-Hospital Care Needed?

  • Struggling with daily tasks after discharge
  • Ongoing pain or limited mobility
  • Uncertainty about medications or instructions
  • Anxiety about falling or coping alone
  • Family members feeling overwhelmed

Early support often speeds recovery and prevents setbacks.
